Health History
A health intake form is completed at or before your first visit to our office. (These forms are available here on our web-site or we can fax, mail or e-mail them to you prior to your first visit.) It is imperative that these forms are filled out completely and accurately because they inform the doctors of your current complaint and your history of injuries and illness.
Consultation
You and one of our doctors will review your health intake forms together. A history of spinal impacts, including auto-accidents, work, birth trauma, recreational and sports injuries will be reviewed, along with your current work and daily activity. You and one of our doctors will also investigate your most pressing health concerns.
Clinical Examination & Evaluation
Your wellness evaluation will include a nervous system examination and a thorough postural evaluation. These tests will help determine if subluxations are affecting your health and body function.
X-rays and Analysis
Your wellness evaluation may require x-rays. The x-rays reveal the degree of spinal misalignment, the phase of spinal degeneration, the presents or absence of significant pathologies, and potential contraindications of care. At Lifetime Family Wellness Centers, careful analysis leads to precision measurements. These measurements along with your clinical exam findings enable us to provide you with individualized treatment and precise and effective care.
